2011-2012 Registration Info   

*June 7th is the first is the first day of New Member Camp, this is when we must have ALL BAND MEMBER contracts and deposits back.  Health form are not due until Pre-Band camp, but are appreciated before this day to make sure all are accounted for and marked clear.

HAVE YOUR HEALTH FORM NOTARIZED BEFORE YOU RETURN IT.  Be sure to fill out and sign the back of the health form before you return it please!

***PLEASE----PLEASE----PLEASE include a copy of the FRONT and BACK of your insurance card attached to your child's health form.

****PLEASE NOTE: 

You MUST turn in a PAPER copy of the CONTRACT FORM as per Shelby County policy

                                                                                    (we must have a signature approving the website usage).
